DSM‑6 to Include Biological Biomarkers, Paving the Way for Precision Psychiatry
The upcoming DSM‑6 will, for the first time, integrate biological markers and neurobiology into psychiatric diagnoses, aiming to enhance precision and reduce subjectivity.

The American Psychiatric Association is preparing to release the next edition of the Diagnostic and Statistical Manual of Mental Disorders, known as DSM-6, which will for the first time include biological factors in the diagnosis of mental health conditions. As a member of the DSM’s strategic committee from May 2024 to August 2026, I contributed to efforts to integrate neurobiology into the manual. This shift reflects growing scientific understanding of the brain’s role in mental illness and addresses long-standing criticism that the DSM previously relied too heavily on symptom-based diagnoses.
The DSM, often called the “bible” of psychiatry, is widely used by clinicians and researchers worldwide to classify mental disorders. However, prior editions—including the current DSM-5, published in 2013—have faced criticism for lacking objective biological measures. Critics argue this approach risks medicalizing human suffering and reduces the validity of psychiatric diagnoses. The forthcoming DSM-6 aims to modernize the manual by incorporating emerging neurobiological insights.
Researchers have yet to identify scientifically valid biomarkers for most psychiatric conditions. The exceptions include certain protein markers used in diagnosing Alzheimer’s disease. Unlike conditions such as diabetes or cardiovascular disease, mental disorders are complex, involving subtle biological changes that current technology struggles to detect. Major mental illnesses like schizophrenia and depression are brain disorders with significant psychological and social influences, making it difficult to isolate purely biological causes.
Mental health conditions are often not discrete illnesses but overlapping syndromes—clusters of symptoms and behaviors that share genetic and environmental risk factors. For example, anxiety and mood disorders frequently co-occur, as do schizophrenia and bipolar disorder with psychosis. This overlap complicates the search for single biomarkers that could reliably diagnose or guide treatment for multiple conditions.
Despite these challenges, advances in neurobiology are beginning to reshape psychiatric diagnosis. One promising development is the FDA’s 2025 approval of a blood test for Alzheimer’s disease, which detects abnormal proteins associated with the condition. This marks the first biomarker-based diagnostic tool for a disorder affecting both brain function and behavior.
Immune factors also show potential as biomarkers. Research indicates that elevated levels of C-reactive protein, a marker of inflammation, are present in about 30% of patients with depression. This suggests that inflammation may help identify a subtype of depression that responds differently to treatment.
Genetic research is another frontier. While no single gene causes mental illness, multiple risk genes contribute cumulatively to disease likelihood. Scientists are exploring “polygenic risk scores,” which combine the effects of many genetic variants, to estimate an individual’s risk for conditions such as schizophrenia or bipolar disorder. Though not yet ready for clinical use, these scores could support more precise, personalized approaches to care.
The DSM-6 will not solve all challenges in psychiatric diagnosis. It will, however, provide a structured framework for incorporating new biological evidence as it emerges. Over time, this could lead to more accurate diagnoses and tailored treatments, moving psychiatry closer to the precision medicine model used in other fields of medicine.
The integration of biology into the DSM represents a significant step forward. While the path is complex, the goal is clear: to bridge the gap between brain science and clinical practice, offering patients and families better tools for understanding and managing mental health.
